Reflux: Survival Guide

 Gastro-oesophageal reflux occurs when acid from the stomach and food moves from the stomach backwards into the oesophagus. It is unknown why some children have symptoms such as pain from the inflammation and others do not, but it is thought to be caused by a number of factors acting together.  

IMPORTANT: Discuss with your GP or paediatrician treatment options!!

Some of these factors include:

-Lack of oesophageal control ie. Relaxation at the incorrect time.
-Abdominal pressure increasing due to crying, coughing, overfeeding, slouching, straining or sitting after feeds.
-Conditions which cause the muscles or nerves to abnormally function.
-Food sensitivities.
-Genetics or hereditary factors.

Medical intervention is sometimes necessary which involves testing and treatment based on the findings- consult you GP or paediatrician.

So what are the symptoms and signs of Reflux?

Usually a baby will not display all of these signs and it is important to note that the number of symptoms do not indicate the severity of the condition. Another note: the signs and symptoms in this list can also be due to other causes so do not assume that the only cause is reflux and seek advice from a health practitioner.

1.    Feeding problems: arching of the back, refusing to feed despite being hungry, crying after or during a feed or even feeding frequently

2.    Hiccupping repeatedly

3.    Crying, screaming or general irritability

4.    Sleeping problems: frequent waking, easily disturbed sleep or difficulty settling

5.    Nasal congestion or recurrent upper respiratory infections

6.    Possetting/vomiting or choking/gagging

7.    Hoarse cry

8.    Excessive salivation in the absence of teething

9.    Weight gain/loss or slow growth

Tips for Managing Reflux

These are some common strategies. If the symptoms persist or are severe, your doctor may suggest referral to a paediatrician or medication.

-       Avoid overfeeding

-       Try a probiotic for baby and Mum. This can help increase the good bacteria and settle the reflux (Bioceuticals makes a great one!)

-       Feed bub in an upright position and following the feed try to keep them upright for at least 30 minutes. (Laying them flat may aggravate the problem at this time).

-       Try to avoid bouncing your baby especially after a feed.

-       Tight clothing especially around the waist is to be avoided. This includes nappies and waistbands.

-       Before the feed, try and change nappies and have tummy time rather than after the feed.

-       Sometimes there may be food sensitivities present. In this case consult a health practitioner before changing your child’s diet or mum may consider an elimination diet for herself.

-       Use of a dummy can sometimes help settle the baby.

-       Some food and drink cause more irritation and increase aciditiy. These are to be minimised eg caffeine, spicy foods, citrus, tomato for Mum and bub.

-       It is still important to follow safe sleeping guidelines.

-       Thickened feeds are effective in some cases where vomiting is a common symptom. Discuss with your GP or paediatrician how to introduce thickeners.

Here are a few survival tips:

-There is no right or wrong way to deal with the problem. Each child is different and therefore often treating the problem will include some trial and error.

-Do not be too hard on yourself. Try and focus on what you are doing right because often it is just not having worked out the best answer for the problem yet.

-Realise that most people will not understand but find a support group that have had to deal with similar episodes in order to share ideas and not feel alone in this – you aren’t alone!

-Trust your instincts- reflux is a medical condition and is not in your mind. Often my clients tell me they were worried that the screaming is just the way their baby is.

-Reflux can be triggered by various factors such as humidity, colds, teething, routine changes, tiredness.

-Look after yourself. It is easy to forget about your wellbeing while dealing with sleeplessness. You are better equipped to find solutions when you are rested.

A little info on the importance of keeping the body during pregnancy and adapting to changes in the body

Research
Another reason to eat more berries! Research is finding that they help clear toxins from the brain. In 2011, the SMH printed an article on the benefits of berries which highlighted specific benefits relating to different types of berries. It is well worth the read.

Dry Needling
I am now offering dry needling both as an adjunct to treatment and on its own. It works really well for shoulder complaints, golfers or tennis elbow and muscle/ligaments sprains and strains.

Xmas Presents
Struggling for Xmas present ideas? I will be ordering in some more Massagers, Pillows and Heat Packs but only if requested. I also have heaps of quirky ideas....

Recipes
I loved this post of meals in a bowl. They all look delicious, healthy and relatively easy to make. I am definitely going to try some of them. Let me know if you do too.

Flood Street Carousel is definitely not new but it took me this long to find it sadly. I've now been going there for their sandwiches which are really lovely! It is also family friendly.
The Pig and Pastry Cafe opened a few months ago around the corner. I haven't yet been but I have heard good things so far. Check it out and let me know what you think!

Preventing Sports Injuries in Children

Preventing sports injuries in your children from an early age will make an amazing difference to the rest of your child's life. As it is, general stresses in life, such as crawling, learning to walk and posture, have a huge impact on our bodies, not to mention the repercussions childhood injuries have when they creep up on us later in life. On a regular basis, adult patients tell of how their falls off bikes, horses or trampolines have caused long term irritation of the spine or how their ankle or knee was never the same after the soccer or netball injury they experienced as a child. 
This doesn't mean that our children shouldn't play sport, in fact it is greatly encouraged as movement is the greatest stimulator of the brain and maintains a healthy body. It does however mean being observant as an adult and listening to your child when they have a complaint. 

Why Are Children Susceptible to Injury?

As any parent knows when buying shoes or clothing, children's bodies are constantly growing and changing. Along with physical change comes internal growth of bones, muscles and cartilage. Of particular concern are the injuries to the growth plate and end of bones. In sports where repetitive training is required, children are extremely vulnerable to stress fractures and injuries caused when the major tendon and ligament insertions pull away from the bone. Children also have no fear!

How Can I Prevent My Child From Injury?

First of all, listen to your child!
Many serious injuries are simply dismissed as 'growing pains'. When your child complains of pain, first thing to do is to make them rest. If there is still pain, it's time to see a health professional. 
Treatment of Injury by your Chiropractor, Physio or GP may be beneficial to get your child back on the field.

Baby Wearing: A Chiropractic View

There has been much talk about the best way to carry a baby. We see often people with bubs in strollers or being carried in a variety of car seats but many parents are now deciding to ‘wear’ their babies. With so many options available, what is the best option for their child? From a chiropractic perspective, here is some insight.

Structure impacts on Function

In Chiropractic, we often say that structure determines function of the body. This means the spine determines the nervous system function. Furthermore, since the nervous system controls the body as a whole, a structurally stressed nervous system will affect the body’s ability to be healthy. Repetitive bad posture can significantly impair our spinal structures. This means that new parents need to be aware of how they choose to carry their little ones as not only can it impact on their spine but on their child’s.   

Baby Carrying and the Importance of Changing Baby Positioning

Car seats are being used more commonly as carriers outside of the car. Often I see Mums’ struggling to remove the seat from the car, twisting and stressing their spine and then tilting sideways when walking holding the carrier.  This activity repeated everyday can lead to injury. Additionally, bub’s body may also be experiencing postural strain in a semi-reclined position. Strollers are also used so commonly and again pulling it out of the car or dismantling it with or without hands full can put undue stress on the spine. Additionally, they prevent the close contact between mum and bub, which in those early days to months can aid in development. A baby spends 9 months in close contact with mum experiencing the warmth, movement and of course contact, therefore the separation after birth may be stressful for the infant.

Babies seem to be spending more and more time on their back. Combined with increased use of strollers, car seats and carriers which only allow a reclined position, there are increased rates of plagiocephaly (flattened head). Due to the head being so soft at birth and for the first few months, it is important that the infant is able to change position regularly during waking hours, including whilst being carried. The flattening of the head causes cranial distortions. Additionally, the use of reclining carriers, do not allow the infant to activate and strengthen their neck muscles.

With these concepts in mind, the best choices for bubs under 4 months are wraps, slings, pouches and sacks, which allow for different positioning for bub. This variety in carrying styles is also important for mum, as the hormone relaxin may stay in the system for up to 9 months. This means the joints are more lax for this time and are more prone to injury.

What about baby carriers

When bubs are able to keep their head up on their own and become a little more curious, baby carriers become a great tool. In this case, it is important to have the little one facing towards you with legs drawn up more than 90 degrees (in a flexed position) and turned out 90 degrees (abducted). This decreases the stress through their pelvis and hips and allows for optimal development of the hip joint. When facing outwards, because of the shape of the carrier, the hip capsule is stretched and the torso is strained making this carrying position less desirable. Often changes in hip development from years in the carrier in an incorrect position are only picked up when the infant learns to walk.

Recommendations when finding a sling or wrap

  • Does the sling allow for various carrying positions for the baby on the wearer?
  • Does the carrier allow for different positions for the baby:  for example; forward or chest facing, vertical, horizontal, folded legs, straight, or frog-leg position? All of these will help with the baby’s postural development.
  • How long will the sling/wrap/pouch accommodate for the child’s growth and postural development?
  • Can the infant be moved from one wearer to another without disturbing them and can they be removed without disturbing them?
  • Is breastfeeding possible while being carried?
  • Is cleaning of the carrier easy?
  • Does the wearer have both hands free or does the baby still require some support with a hand?
  • Can the baby be put into all carrying positions by the wearer on their own, or do they require help from a second person?
  • Is there even distribution of the baby’s weight for the wearer’s comfort?
  • Does the sling/wrap cause repetitive postural stress to one area of the wearer’s spine?
Recommendations when choosing a carrier:

· Make sure there is head and neck support for the baby and/or sleeping infant

· The carrier should have a wide base of support that supports the thighs and flexes the knees

· Try and find a carrier with postural curve support to avoid stressing the infant’s spine

· Wide and/or cushioned adjustable shoulder straps will allow for easier carrying and less strain on the Carriers neck and shoulders

· Make sure the carrier gives you the ability to distribute the baby’s weight evenly across your body

· The carrier should allow for ease of loading/unloading, so that the you can keep your spine in a neutral, safe and comfortable position.

Research:
Here is a research study I came across last week regarding DHA, a component of fish oil. The study was testing the effectiveness of supplementation to help with memory and reaction times. Interestingly, they noted improvements in both of these categories in women and men respectively! Well worth the read!

Recipe:
I speak a lot about trying to be more alkaline than acidic when eating to try reduce the inflammation, boost energy as well as to increase the metabolism of nutrients. Drinking a green juice/smoothie in the morning is a good way to help in this alkalising process. 
I usually use: 1 lemon juiced, 2 peeled oranges, a cup of baby spinach leaves, a handful of parsley and sometimes an apple.

Why would a child need chiropractic?...from falls when learning to walk, bumps on the head, slips, bike/trampoline injuries when playing sport and carrying heavy school bags causing neck or shoulder tension.  

Treatment

The common concern of many parents is that chiropractic adjustments will be too forceful or the same as adult treatment. The adjusting technique is always modified depending on the size of the individual and their particular problem. Additionally, a child’s spine does not have the same tightness and repetitive postural stress that we commonly see in adults. This allows for kids chiropractic to be extremely gentle and comfortable.

Chiropractic

Chiropractors study for 5 years, learning, understanding and training in specific techniques for kids.  These techniques can be used from a 1-day old baby upwards. The chiropractic adjustment itself is very specific and uses no more pressure than used to test the ripeness of a tomato.
